Background checks/drug screen

To enroll as a hospice volunteer, you need to submit paperwork for KP to run a background check and request a drug screening.

Per Medicare requirements, KP Hospice Volunteer Program will need to run a background check (only) every two years.






KP volunteer badge

Take a picture of yourself against a blank, light colored background. Submit to hospice.

Hospice will send you a your volunteer badge after you complete the training.

  1. Badges must be worn during every hospice assignment

  2. If you loose your badge, contact your Volunteer Coordinator ASAP


Employee Health/Enroll in ready set

You will need to enroll in our Employee Health database, called Ready Set with your personal health information. To enroll, you will receive an email with your ID# and instructions on how to enroll.

To enroll:

  1. Click the link below to access your secure and private MyHealth webpage online:

  2. Follow the directions. If you get stuck, contact your Volunteer Coordinator immediately.

  3. Save your log on username and password. You will receive communication from KP Employee Health about vaccines or flu shots. You can upload documents regarding your personal health information in Ready Set.
